**Title** Physical Therapist – Rehabilitation **Schedule** 8 hours **Assignment Details** Contract Length: 14 weeks Guaranteed Hours: 40 **Requirements** - Current State License as a Physical Therapist - Required - Proof of current CPR certification - Required - Current driver's license with reliable transportation and insurance - Required - 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required - Previous home care experience - Preferred - Bachelor of Science in Physical Therapy from an APTA approved program - Required **Roles & Responsibilities** - Perform clinical assessments to identify patient needs and establish appropriateness for therapy. - Document and complete Plans of Care (POC), including progress notes and other clinical documentation on an ongoing basis, revising as necessary. - Plan and implement treatment programs for patients with neuromuscular, neurological, and orthopedic disorders based on medical referrals and evaluations. - Educate patients and families on therapeutic procedures and safe use of assistive equipment. - Collaborate with an interdisciplinary team and consult with physicians regarding patient care. - Adhere to infection control procedures and universal precautions while providing patient care. **Compliance / Notes** - Must meet mandatory continuing education requirements. - Ability to work under stressful conditions meeting patient needs.
