*Position pays $90000 - $135000 per year (depending on experience)!

Key Responsibilities
- Develop and implement individualized care plans
- Provide therapy services per physician’s orders
- Assess functional status and recommend adaptive equipment
- Teach new skills or retrain patients in daily activities
- Maintain accurate documentation and participate in case conferences
Requirements
- Licensed Occupational Therapist (OT) in the State of Arizona
- Graduate of an ACOTE-accredited OT program, eligible for NBCOT certification
- Minimum 2 years’ clinical experience (home health preferred)
- Current CPR certification
- Valid driver’s license, insurance, and reliable transportation
